      • The request should include the Named Insured, Policy Number, Certificate holder's name & address, an explanation of the services provided by the insured to the certificate holder and where the certificate should be sent via mail, fax or email.

  • Cancellation – Submit LPR with insured’s signature, reason, and cancellation date.
  • Reinstatement Request - The insured must call The Hartford directly for approval and to arrange for payment at (800) 962-6170.

    • Please provide a COI for...The request should include the Named Insured, Policy Number, Certificate holder's name & address, an explanation of the services provided by the insured to the certificate holder and where the certificate should be sent via mail, fax or email.

Broker of Record Requests – Must be submitted on insured’s letterhead and include the policy number, effective date, a request naming BOTH Number One Insurance and your agency as the Broker of Record and the insured’s name, signature, and title.
